Procedure

1. The Application for Safety License form is available with the GMA Safety Inspectors at the Landing sites for free;
2. The Application for Safety License forms shall be submitted to the GMA Safety Inspectors for surveying the boat. The survey result will be used to determine the allocation of a Safety License or referral for remedying deficiencies if found on the boat; and
3. The fee for the Safety License is in accordance with the GMA-approved charges schedule.

Requirements

1. Applicant form (Safety Inspectors, Online /mis);
2. Safety License Application form (D300);
3. Valid National ID Card of Applicant;
4. Gross tonnage of Ship;
5. Name of Agent;
6. Place of Survey;
7. Capacity of Ship (GT or number of passengers);
8. Destination;
9. Overall Length; and
10. Type (Fishing or Cargo).
